N4t Investigators: Inside Sapphire Of Tucson
TUCSON – Weve brought you the stories of terrified family members. Their loved ones living inside one of Arizonas hardest hit long-term care facilities that has been consumed by COVID-19. Now, we are sharing stories from inside of the facility, as those who live and have worked inside Sapphire of Tucson, speak out.
“It got to the point where I didn’t feel safe,” said former Sapphire of Tucson employee, Kayla Dennis. Dennis has been a certified nursing assistant for 14 years. Until recently, she took care of the residents inside Sapphire of Tucson, a long-term care facility. She said she loves the staff and patients, but believes that the care facility is not prioritizing the health of either group and she feels that they are putting them in harms way.
Dennis said, “they would admit patients to where we were overwhelmed. She added, that there was not enough staff to handle the workload on a given day and “sometimes I would have 10 patients and sometimes I would have 20 patients.”She says that was before the coronavirus hit the rehabilitation facility.
Dennis took a seven week leave of absence on March 14, just before the first patient at the facility was diagnosed with COVID-19. “Patients started getting fevers, and were acting funny. We told the nurses and they wouldn’t listen to us,” she said. Dennis said as much as she hated to leave her patients, she had to leave the facility to take care of her disabled daughter and elderly father-in-law.

One of the reasons this proved to be a decent nursing home is it received a favorable inspection score. In this category, we awarded this facility a B+. Inspection grades are based on pieces of information located in the a nursing home’s recent inspection reports. Places that receive favorable grades in this area typically have very few deficiencies on their reports. Most importantly, these nursing homes generally do not have any severe deficiencies which are associated with endangerment of patients. This place was hit with 5 deficiencies on its inspection report, but none were considered to be major deficiencies. This indicates that the government inspectors didn’t consider any of these deficiencies to pose an immediate threat to resident safety or health. A couple minor deficiencies shouldn’t lead you to cross a nursing home off your list.
Sapphire Of Tucson Nursing & Rehab At The Center Of Tucson’s Covid 19 Cases
Posted by | Apr 16, 2020 | 0 Comments
A skilled nursing center adjacent to Banner Kino Hospital and the Kino Sports Complex has become Tucson’s Ground Zero for novel corona virus cases. Sapphire of Tucson Nursing & Rehab is a 240 bed facility that provides nursing care and rehabilitation services to patients.
On March 30, 2020 news broke that over 2 dozen COVID-19 cases were reported out of Sapphire. Given the testing guidelines and limitations, and the reality that asymptomatic individuals may be infected, the likely number of cases is higher.
Already families of Sapphire patients are telling their stories of alleged mismanagment of the situation and poor communication. KVOA’s Lupita Murillo shared the story of the Ard family, and KGUN 9 reported on another family’s experience.

As her father neared death, Leslie Munsey drove through the middle of the night to say goodbye.
Victor Munsey had been in the hospital since March 30 after falling out of bed. There it was discovered he had contracted the coronavirus at Sapphire of Tucson Nursing and Rehabilitation, where he lived because of his dementia.
He was placed on a ventilator, and doctors advised Munsey on May 1 that his condition was worsening. The Sierra Vista resident borrowed her neighbors Hyundai Elantra and headed to Hereford at 2 a.m. so she could use her daughters iPhone to video call him.
From the driveway, Munsey told her father she loved him.
I told him, If you want to go, you can go, but if you want to stay, you have to fight. If you want to go, I wont be mad at you. I know you want to be with Mom, she said.
He died 13 hours later. He was 65.
Newly released data shows that 33 Sapphire residents, including Munsey, have died from COVID-19 since the start of the outbreak in March. That ranks the highest among federally licensed nursing homes in the state, new federal data shows.

Woman Claims Mother Was Sexually Assaulted By Employee At Tucson Nursing Home
TUCSON, Ariz. – An investigation is underway at a Tucson nursing home after a woman who lives at the facility reported she was sexually assaulted by an employee.
Obviously, you cant trust anybody,” said Selina Poss, the residents daughter.
Poss said her 63-year-old mother receives around-the-clock care at Sapphire Tucson Nursing and Rehabilitation, 2900 E. Melber Street.
While she said the family had issues with her mothers care before, it was a call over the weekend that stopped her in her tracks.
During the phone call, Poss said her mother told her she was sexually assaulted by a nurse tech she was unfamiliar with. The resident said, according to Poss, the man put his genitals in her hand and then started inappropriately touching her.
He told her, This is our secret,'” Poss said.
Poss claimed her mother told other employees what happened before their phone call, but nothing was done.
They didnt call. To me, an incident like that, you call somebody and then you get that person to the hospital so they can be tested,” Poss said. “I mean, who knows how many other females hes done this to?
The Tucson Police Department confirmed a report was made Sunday, June 23. According to a case management document provided by Poss, a detective was assigned to the investigation Tuesday, June 25.
Poss said she is frustrated over the lack of help by the facility.
Poss said she wont stop fighting for an answer. Not just for her mother, but for other residents at the facility.
