In addition to receiving a great overall score, this facility also received an excellent short-term care grade, with an A. Short-term care scores are crucial for individuals needing rehabilitation. Rehabilitation generally mandates additional highly-skilled nursing services. Skilled nursing includes a broad range of services, spanning from registered nurses to physical and speech therapists, as well as other forms of therapy. This nursing home employs both physical therapists and registered nurses. Not all facilities employ these skilled professionals. Lastly, we assessed the number of residents who were able to return to the community from this facility. This proved to be a real strength for this facility. It outperformed the vast majority of nursing homes in the nation in this area with 64.5 percent of its patients returning home.

The grounds are well maintained except the hill behind my house near the employee parking. There is trash including used masks, dead trees and tree branches, dandelions which have now spread to my yard. The ugly black tube that was supposed to keep water from running down the hill, which it has not. This has caused erosion in my yard and every time it rains I have a swamp. They have refused to repair the damage and continue to ignore and not respond.

My father and sister have both been to Nazareth. I cannot thank them enough for their compassion and care. in admissions went over and above in helping us deal with the financial end and the social worker went over and above in her care and attention to empathetic detail.
My husband recently spent 1 month in rehab at Nazareth. The staff was excellent, friendly, and went out of their way to make sure he was included in social activities. Never once did I see anyone who didn’t look like they enjoyed being there. My husband raved about the food and made wonderful progress with the physical and occupational therapists on staff. I would highly recommend Nazareth to my family and friends. The facilities were clean, bright, and amidst several safe, family-oriented neighborhoods.
We were going to take a tour of the community, but as soon as we pulled up to the community my loved one said no absolutely not. My loved one did not like that the community was isolated from everything and every body. My loved one said that it just wouldn’t work out.
